Essex Partnership University NHS Foundation Trust is seeking an experienced Senior Community Nurse / District Nurse to join their dynamic multi-disciplinary care team. This is an exciting opportunity for a motivated healthcare professional to make a significant impact on community healthcare delivery in West Essex.

Key Responsibilities

The successful candidate will be responsible for providing highly skilled nursing care to residents of West Essex, promoting public health, reducing the effects of ill health and disability, and maximising individual independence. The role encompasses comprehensive assessment of patient/family needs, discharge planning, and the development, implementation, and evaluation of care programmes.

You will initiate palliative care packages with multi-agency providers, work as an autonomous practitioner within the Community Nursing Team, and support the delivery of care in patients’ own homes. The position requires close collaboration with system partners, Primary Care Networks, and Neighbourhood Teams as part of the out-of-hospital model of care.

Specific Duties Include:

  • Undertaking comprehensive and complex patient assessments
  • Leading the day-to-day management of community staff nurses
  • Supporting, directing, and teaching junior team members
  • Maintaining accurate nursing records according to NMC guidelines
  • Organizing patient care with healthcare professionals and agencies
  • Working within a 7-day a week, 365-day a year service model

  • Registered General Nurse with current NMC registration

  • District Nurse Qualification
  • Non-Medical Prescribing qualification (V100/V300)

  • Compression Bandaging Skills
  • Palliative Care experience
  • Catheter care experience
  • Wound management expertise
  • Proven leadership and communication skills
  • Ability to manage a caseload effectively

Desirable

  • Previous experience at Band 6 level

  • Continuing Healthcare knowledge and assessment experience
  • Experience working within a multi-disciplinary team

Benefits and Compensation

This permanent, full-time position offers a competitive salary ranging from £38,682 to £46,580 per annum, plus 5% HCA (High Cost Area) allowance. The Trust provides an excellent pension scheme of up to 14.5% of pensionable pay and a comprehensive benefits package.

Working Environment
